Register on this sheet all students who have been in your school for the last twelve months or who completed the highest grade at their former school last year.

4. Number of credits earned at conclusion of previous school year.

5. Did the student participate in a TSSAA contest in the last twelve months (Varsity, B-Team, or Junior High) in this sport?

6. Has the student been enrolled in your school for the last twelve months?

7. Is the student living at home with parents or if they are living with a guardian other than a parent or parents, have they lived with them a minimum of twelve months?
